I can't do this alone! If you feel that an entry's description is lacking, or if I missed something totally obvious, let me know.
There is some stuff I couldn't find official names for. Please take a moment to glance through the pictures/descriptions and see if you can help me. If you do have the official name for something, please mail me its official name, a description of the item, and where you found the name (if possible).
If you want to really help me out, try and get me a copy of the manuals for: Yoshi's Safari, VB Wario Land, Mario Tennis, and Mario Clash. I'm hoping they list at least some enemy/item names.

Discrepancies
Usually the first name a Nintendo publication gives to a Mario item is the one used in the encyclopedia, although there are some exceptions...
- Mario Kart 64 items
Nintendo Power magazines use a plethora of different names for MK64 items, but names used in the encyclopedia are the names listed on the MK64 Operation Card (packaged with the game). Why similar items did not retain their original SMK names, I do not know.
- Shy Guys
SMB2 calls them "Shyguys," SMW2 calls them "Shy-Guys," and every other game calls them "Shy Guys." Grr! Anyway, they're all the same enemy, so I classified them under "Shy Guys."
- Super Mario Bros. 3 items
The "Magic Whistle" and "Magic Switch," as named in the SMB3 instruction booklet are "Warp Whistle" and "P-Switch" in the encyclopedia. I felt that the last two names would be more popular, because every Nintendo coverage of SMB3 published after that refers to them as the latter names.
- Super Mario Bros. 2 (Japanese)
Although I don't have proper documentation, all SMB2(J) enemies and items are named the same as their SMB counterparts. It's rare that I make up names for stuff, but the graphics in SMB and SMB2(J) are exactly the same...
- Super Mario Kart
It's "Flopping Cheep Cheep" in the manual, but I listed it as "Cheep Cheep" because I didn't want to do another enemy entry :P

- Mario Kart 64
Bats, penguins, porcupines, snowmen.
- Super Mario Kart
Bowser's fireballs.
- Super Mario Land 2: 6 Golden Coins
Most everything.
- VB Wario Land
Stuff not in instruction manual.
- Wario Land: Super Mario Land 3
Stuff not in instruction manual and Nintendo Power 58.
- Wario Land II
Stuff not in instruction manual and Nintendo Power 106.
- Wario's Woods (NES)
Every boss except Wario.
- Yoshi's Safari
All items, and all enemies except the Koopa kids.
